Job summary

This full-time teleworker role requires residency in WV and involves substantial travel to central WV locations, including homes and facilities, during standard business hours.

Qualified candidates have at least 2 years in behavioral health, strong MS Office skills, and reliable transportation.

Qualifications

• Minimum 2 years in behavioral health or related field.
• WV resident residing in one of the specified counties.
• Willing to travel 50% or more in the assigned region; mileage reimbursed.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint, Teams).

Responsibilities

• Conducts comprehensive evaluation of referred member's needs/eligibility and recommends a plan.
• Identifies high risk factors and coordinates care plan progress with team.
• Coordinates and implements care plan activities and monitors progress.
