Ideal candidates have 2+ years in behavioral health or related field, strong MS Office skills, and reliable transportation for frequent regional travel.

Qualifications

• 2 years’ experience in behavioral health, social services, or related field**.
• West Virginia resident in one of Barbour, Brooke, Doddridge, Hancock, Harrison, Lewis, Marion, Marshall, Monongalia, Ohio, Pleasants, Ritchie, Taylor, Tyler, Upshur, Wetzel counties.
• Must possess reliable transportation and be willing to travel 50%+; mileage reimbursed per policy.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint, Teams).

Responsabilités

• Collaborate and inform the case management process to facilitate healthcare outcomes.
• Evaluate member needs and benefits; coordinate care plans.
• Travel to member homes, residential facilities and other sites as required.
