CVS Health is seeking a full-time, remote field teleworker in West Virginia to evaluate members, coordinate care and resources, and monitor care plans. The role requires 50%+ travel in the central WV region, with residency in Braxton, Calhoun, Clay, Fayette, Gilmer, Kanawha, Nicholas, Roane, or Webster counties.

Qualifikationen

• 2 years' experience in behavioral health, social services, or related field
• West Virginia resident in listed counties
• Must travel 50%+ of the time; mileage reimbursed per policy
• 2+ years of experience with PC, keyboard, mouse, and MS Office Suite (Outlook, Word, Excel, SharePoint, Teams)

• Identify high risk factors and coordinate with clinical teams for care planning.
• Monitor and document care plan progress and outcomes.
