• WV resident in designated counties and must have reliable transportation.
• 2+ years in behavioral health or related field.
• Proficient with MS Office applications (Outlook, Word, Excel, SharePoint, Teams).

Responsabilidades

• Provide care coordination and education to members.
• Collaborate with case managers and Medical Directors to optimize care.
• Evaluate member needs, eligibility and benefit plans and coordinate care.
• Document and monitor progress in care management systems.
