Job summary

CVS Health is seeking a full-time field teleworker based in West Virginia who will evaluate member needs, coordinate care and support access to benefits. The role requires residency in WV, travels 50% or more in the central region, and works with member homes and facilities.

You will collaborate with care managers, medical directors and clinical staff to improve outcomes, document progress, and ensure regulatory compliance while delivering compassionate, patient-centered care.

Qualifications

• 2 years’ experience in behavioral health or related field.
• West Virginia residency in Braxton, Calhoun, Clay, Fayette, Gilmer, Kanawha, Nicholas, Roane, Webster-Regional
• Willing and able to travel 50% or more of the time in the central WV region.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint, Teams)

Responsibilities

• Through care management tools and data review, evaluate referred member’s needs/eligibility and recommend approaches.
• Identify high risk factors and service needs affecting outcomes and refer to clinical case management or crisis intervention.
• Coordinate and implement assigned care plan activities and monitor progress.

Bachelor's degree in behavioral health or human services

Non-licensed master's degree in behavioral health or human services
