Qualifications

• 2 years' experience in behavioral health or equivalent field.
• Must reside in WV in Braxton, Calhoun, Clay, Fayette, Gilmer, Kanawha, Nicholas, Roane, or Webster-Regional counties.
• Must have reliable transportation and travel 50%+ of the time; mileage reimbursed.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint, Teams).

Responsibilities

• Evaluate referred members' needs and benefits to plan care.
• Identify high risk factors and coordinate care planning with clinicians.
• Monitor and document care plan progress and outcomes.
