Job summary

CVS Health is seeking a Case Management Coordinator for a full-time telework position with 50-75% travel around Cook County, Illinois, primarily the west side of Chicago. The role supports members by coordinating care plans, scheduling appointments, and helping members access benefits.

You will collaborate with care managers, medical directors and health programs to achieve optimal outcomes, document care, and ensure regulatory compliance.

Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Willing and able to travel 50-75% around Cook County, IL – primarily west side of Chicago.
• 2+ years of electronic documentation experience and proficiency with Microsoft Office applications.

Responsibilities

• Evaluate members’ needs/eligibility and recommend care resolution paths.
• Identify high-risk factors and coordinate appropriate referrals.
• Coordinate and monitor care plan activities and progress.
• Consult with care managers and Medical Directors to overcome barriers to goals.
• Escalate quality of care issues through established channels.
• Negotiate options to meet member benefits and healthcare needs.
• Engage members to promote healthy lifestyle changes and participation in decisions.
• Provide coaching and information to empower independent medical decisions.
