Job summary

CVS Health is seeking a Field Case Management Analyst to coordinate care and educate members, employing motivational interviewing to promote healthier choices. This full-time role requires travel within 60 miles and evening availability, with a focus on compliant documentation and collaborative care planning.

Ideal candidates hold a Bachelor's and possibly a Master's in healthcare or related fields, with CCM certification preferred.

Qualifications

• Minimum of two years in case management or related field.
• Experience with adult and/or adolescent populations.
• Excellent verbal, written, and telephonic communication skills.
• Demonstrated motivational interviewing skills and ability to establish rapport telephonically.
• Strong documentation skills and attention to detail.
• Commitment to compliance and quality standards.
• Willingness to travel up to 50% within 60 miles one way.
• Must reside in Louisiana.
• Ability to work a variable schedule including two evenings per week until 9:00 PM local time.

Responsibilities

• Conduct comprehensive assessments to evaluate member needs, eligibility, barriers to care, and opportunities for intervention.
• Provide routine care coordination, support, and education to improve health outcomes.
• Utilize motivational interviewing techniques to increase member engagement.
• Empower members to participate in their care plans and make informed decisions.
• Collaborate with case managers, supervisors, medical directors, and providers to address needs.
• Present cases at interdisciplinary conferences to support optimal outcomes.
• Maintain accurate, timely, and compliant documentation of activities.
• Meet productivity, quality, and performance metrics while managing a diverse caseload.
• Support implementation of case management programs and processes.
