The role requires strong communication, motivational interviewing, documentation, and the ability to manage a diverse caseload while achieving metrics and regulatory standards.

Qualifications

• 2+ years in case management or related field.
• Experience with adult/adolescent populations.
• Strong customer service and communication skills.
• Telephonic case management experience preferred.
• Ability to travel up to 50% within 60 miles.

Responsibilities

• Conduct comprehensive assessments of member needs and barriers to care.
• Provide routine care coordination, support, and education.
• Use motivational interviewing to engage members.
• Collaborate with care teams and providers to address needs.
• Present cases at interdisciplinary conferences to support outcomes.
• Document all activities accurately and timely.
• Meet productivity, quality, and performance metrics.
• Support implementation of case management programs and processes.
