Job summary

CVS Health’s Aetna team seeks a Case Management Coordinator to support Medicare/Medicaid members, coordinating appointments and benefits access to improve health outcomes. This full-time, home-friendly role requires collaboration with clinicians, travel within the designated area, and use of case management tools while upholding regulatory standards.

You will engage with members to promote healthy lifestyle choices, document care activities, and participate in team meetings.

Qualifications

• Bachelor's degree in behavioral health or human services (or equivalent).
• Experience in case management and discharge planning preferred.
• Ability to travel within a designated geographic area for in-person activities.
• Strong analytical and problem-solving abilities.
• Excellent communication, organizational, and interpersonal skills.
• Proficiency with MS Word, Excel, Outlook and PowerPoint.

Responsibilities

• Coordinate and monitor care plans, schedule appointments, and empower members.
• Identify and assist with accessing benefits and education for members.
• Collaborate with case managers and health teams to achieve optimal outcomes.
• Engage with members to support healthy lifestyle choices and adherence.
• Document care processes in accordance with regulatory and company policies.
