Job summary

Humana’s Inclusa team in Wisconsin seeks a Case Manager to deliver comprehensive care coordination for frail elders and adults with disabilities in Dane and Columbia counties. You will collaborate with a Field Care Nurse, develop and update the Member Care Plan, and conduct in-person assessments and visits as part of a patient-centered care model.

Role requires a Bachelor's degree in human services (or related) with relevant experience, and travel up to 40% with a valid driver's license.

Qualifications

• Bachelor's degree in human services or related field with at least 1 year of relevant experience; or Bachelor's in another field with at least 3 years of relevant experience.
• Willingness to travel up to 40% of time and maintain a valid driver’s license with required insurance.

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan.
• Coordinate with Field Care Nurse to address needs in least restrictive environment.
• Travel to member locations for assessments and quarterly visits.
• Maintain monthly contact with members by phone.
• Arrange non-medical supports such as housing, employment, and social resources.
• Ensure cost-effective service delivery and accurate MCP documentation.
