Responsibilities

• Assess members' health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs.
• Travel to members' locations for assessments and visits (up to 40%).
• Arrange support services for social integration, community resources, housing, and other non-medical needs.
• Maintain accurate documentation and updates to the MCP.
