Job summary

CVS Health is seeking a Case Management Coordinator for a full-time remote role involving substantial travel in Illinois. You will help members access benefits, schedule appointments, and navigate care options to improve health outcomes.

You’ll work with care management tools, collaborate with medical directors and other health teams, and coach members toward sustainable healthy lifestyle choices while ensuring regulatory compliance.

Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Willing to travel 50-75% in Mason, Cass, Scott and Brown counties, IL.
• 2+ years of electronic documentation experience and proficiency with Microsoft Office.

Responsibilities

• Evaluate members’ needs and eligibility and recommend a care plan approach.
• Identify high risk factors and coordinate with clinical teams.
• Coordinate and monitor care plan activities and progress.
• Consult with case managers/Medical Directors to overcome barriers to goals and ensure quality of care.
• Identify and escalate quality of care issues through established channels.
• Negotiate options and services to meet member benefits and healthcare needs.
• Engage members using motivational interviewing to encourage healthy behaviors.
• Provide coaching and support to empower informed health decisions.
• Participate in team meetings and peer mentoring.
