You will coordinate care, assess needs, and partner with clinical teams to improve health outcomes for LTSS/waiver members. The position emphasizes care coordination, education, and engagement to support informed decision-making and effective use of benefits.

Qualifications

• Bachelor’s degree or higher in a human-services field (e.g., sociology, rehabilitation counseling).
• Minimum two years of case management experience.
• Must reside in Illinois (west side of Chicago area) and have reliable transportation.

Responsibilities

• Coordinate and monitor care plans for waiver/LTSS members.
• Evaluate needs/eligibility and coordinate referrals to appropriate services.
• Engage members to promote health outcomes and empower decision-making.
• Collaborate with clinicians and program staff to ensure quality of care.
