Qualifications

• Must reside in Illinois in the Naperville, Joliet or Aurora area.
• Valid Illinois Driver's license
• Reliable transportation and travel up to 75% of the time (mileage reimbursed)
• Minimum of two years of case management experience
• Microsoft Office and electronic health record experience

Responsibilities

• Conduct evaluation of member needs/eligibility and recommend case resolution or services.
• Identify high risk factors and coordinate appropriate referrals to clinical management or crisis intervention.
• Coordinate and monitor care plan activities and progress.
• Consult with case managers, Medical Directors and health programs to overcome barriers and optimize outcomes.
• Provide coaching and support to empower members in healthcare decisions and healthy lifestyles.
