Qualifications

• Minimum of two (2) years of case management experience.
• Microsoft Office and electronic health record experience.
• Must possess reliable transportation and be willing to travel up to 75% of the time.
• Valid Illinois Driver’s license and reside in zip codes or surrounding areas.
