Job summary

CVS Health is expanding its Long-Term Services & Supports (LTSS) initiative with a Field Case Management Analyst role in Illinois. This field-based position requires 50–75% travel to engage members face-to-face and manage full caseloads, including quarterly in-person visits.

You will coordinate care, educate members, and utilize care management tools to improve outcomes, while partnering with clinical teams to ensure quality and regulatory compliance.

Qualifications

• Minimum of two years of case management experience.
• Valid Illinois driver’s license and reliable transportation.
• Experience with Microsoft Office and electronic health records.

Responsibilities

• Evaluate member needs/eligibility and plan case resolutions.
• Coordinate care plans and monitor progress with interdisciplinary teams.
• Provide coaching and support to empower members in healthcare decisions.
