Qualifications

• Must reside in Illinois, Champaign area.
• Two or more years of case management experience.
• Valid Illinois driver's license and reliable transportation; travel up to 75%.
• Proficiency with Microsoft Office and electronic health records.

Responsibilities

• Evaluate referred members, assess needs, and determine appropriate care planning.
• Coordinate and implement care plan activities; monitor progress and outcomes.
• Collaborate with case managers, Medical Directors and health programs to overcome barriers.
• Escalate quality of care issues through established channels.
• Provide coaching and support to empower members to participate in decision-making.
