• Minimum of two (2) years of case management experience
• Microsoft Office and electronic health record experience

Responsibilities

• Evaluate member needs/eligibility and tailor case resolution and care planning.
• Coordinate and monitor care plan activities for waiver members.
• Identify high-risk factors and make referrals to clinical case management or crisis interventions as appropriate.
• Provide coaching and support to empower members to participate in their healthcare decisions.

Education

Bachelor's degree or Master’s in a human-services field
