Qualifications

• Must reside in Illinois Macoupin County & Surrounding Areas, IL.
• Valid Illinois Driver’s license.
• Reliable transportation and travel up to 75% of the time.
• Minimum of two years of case management experience.
• Proficient in Microsoft Office and electronic health records.

Responsibilities

• Evaluate referred members' needs and eligibility and plan care.
• Identify high-risk factors and coordinate appropriate referrals.
• Implement and monitor assigned care plan activities.
• Coordinate with clinical teams to improve outcomes.
