Job summary

Qualifications

• Two years of case management experience.

• Reliable transportation, mileage reimbursement per policy.
• Experience with HIV/AIDS HCBS Waiver populations if applicable.

Responsibilities

• Evaluate referred members’ needs/eligibility and plan case resolution.
• Identify high risk factors and coordinate with clinical case management for care planning.
• Coordinate and monitor care plan progress and outcomes.
