Job summary

CVS Health is seeking an Analyst, Case Management Field for the LTSS team, primarily working in Rock Island County, IL. The role requires 50-75% travel and in-person member visits to manage waiver members and ensure quality care coordination.

You will evaluate member needs, coordinate care plans, and collaborate with case managers and medical directors to overcome barriers and promote optimal outcomes. A bachelor’s or master’s degree in human-services is required.

Qualifications

• Minimum of two (2) years of case management experience.
• Microsoft Office and electronic health record experience.
• Willing and able to travel up to 75% of time to meet with members face to face.
• Must reside in Illinois in Rock Island area and have a valid Illinois driver’s license.

Responsibilities

• Evaluate referred members' needs/eligibility and determine approach to case resolution.
• Identify high risk factors and coordinate appropriate referrals to care management or crisis intervention.
• Coordinate and monitor progress of assigned care plans and activities.
