Job summary

CVS Health is seeking an Analyst, Case Management Field for the LTSS team, based in the Rock Island County area of Illinois. This field-based role requires 50–75% travel to meet members face-to-face and manage a full caseload.

You will coordinate care plans, evaluate needs, and connect members with internal and external resources to support outcomes. Required: bachelor’s or master’s in a human-services field, two years of case management, Microsoft Office and EHR experience, and a valid Illinois

Qualifications

Must reside in Illinois, Rock Island area.
Valid Illinois driver's license.
Willing to travel up to 75% to meet members face-to-face.
Reliable transportation, eligible for mileage reimbursement.
Minimum of two years of case management experience.
Microsoft Office and electronic health record experience.
If handling HIV/AIDS waiver members, experience with minorities and related communities.

Responsibilities

Conduct comprehensive evaluation of members' needs and eligibility and recommend approaches.
Coordinate and implement care plan activities and monitor progress.
Provide coaching and support to empower member healthcare decisions.
