Job summary

CVS Health is seeking a Case Management Coordinator on the Aetna team to advance patient care for members enrolled in Medicare and Medicaid across Illinois. You will facilitate scheduling, help access benefits, and educate members using care management tools in a collaborative, multidisciplinary environment.

Responsibilities include coordinating care plans, identifying needs, and supporting care team decisions to improve outcomes.

Qualifications

• Must reside in Illinois.
• Must possess reliable transportation and travel 50-75% in Winnebago, Boone and McHenry counties; mileage reimbursed.
• Must be computer literate with Excel and Microsoft Word.
• Effective communication, telephonic and organizational skills.
• Excellent analytical and problem-solving skills.
• Ability to work independently.
• Ability to participate in a multi-disciplinary team.
• 2 years' experience in behavioral health, social services or related field.

Responsibilities

• Evaluation and coordination of care plans to determine member needs and eligibility.
• Identify high-risk factors and refer to case management or crisis services as needed.
• Coordinate and monitor care plan activities and progress.
• Collaborate with Medical Directors and care teams to overcome barriers to goals.
• Identify and escalate quality of care issues through established channels.
• Negotiate options to meet member benefits and healthcare needs.
• Use motivational interviewing to engage members and promote healthy behaviors.
• Provide coaching and education to empower informed medical decisions.
• Document care processes using case management and quality procedures.
