Job summary

CVS Health’s Aetna team seeks a Case Management Coordinator to elevate patient care. You will support scheduling, access benefits, and provide member education using care management tools to inform outcomes.

You will collaborate with members, providers, and community organizations to address social determinants of health while working from a home-based, HIPAA-compliant environment up to 50–75% travel in Illinois and surrounding areas.

Qualifications

• Must reside in Illinois.
• Willing to travel up to 50–75% from home in and around Winnebago and surrounding areas, IL.
• Minimum 3–5 years clinical practical experience.
• Independent and comfortable collaborating virtually.
• Strong analytical, problem-solving and communication skills.
• Proficient with MS Word, Excel, Outlook and PowerPoint.

Responsibilities

• Assist with appointment scheduling and care coordination.
• Identify and assist with accessing benefits and education for members using care management tools.
• Collaborate with members, providers and community organizations to address social determinants of health.
• Maintain HIPAA-compliant documentation and workflows.
