Job summary

CVS Health is seeking a Case Management Coordinator to support members across Illinois counties from a home-based setting. You will schedule appointments, identify benefits access, and leverage care management tools to drive outcomes.

Qualifications

Case management and discharge planning experience preferred
2 years experience in behavioral health, social services or related field
Managed Care experience preferred
Ability to travel within a designated geographic area for in-person case management activities
Excellent analytical and problem-solving skills
Effective communications, organizational, and interpersonal skills
Ability to work independently
Proficiency with MS Word, Excel, Outlook and PowerPoint
Efficient and effective computer skills including navigating multiple systems

Responsibilities

Evaluation of Members: assesses needs/eligibility and recommends care strategies.
Identifies high risk factors and coordinates referrals to clinical care.
Coordinates and monitors care plan activities and progress.
Collaborates with Medical Directors and other health programs to improve outcomes.
Identifies and escalates quality of care issues through established channels.
Negotiates options and services to meet member benefits.
Engages members to promote healthy lifestyle changes.
Provides coaching and support for independent decision-making.
Attends team meetings and mentors peers.
Supports member participation with providers in decision-making.
Documents care and ensures regulatory compliance.
