Job summary

CVS Health is seeking a Case Management Coordinator in Illinois to support Aetna members. You will evaluate needs, coordinate care plans, and assist with scheduling and benefits access, collaborating with care teams to improve outcomes.

The role requires travel across specified counties, strong communication, and independent work to ensure comprehensive, person-centered support for members and families.

Qualifications

• Must reside in Illinois.
• Must be willing to travel 50-75% around Winnebago, Boone and McHenry counties, IL.
• Computer literacy including Excel and Microsoft Word.
• Effective communication, telephonic and organizational skills.
• Excellent analytical and problem-solving skills.
• Ability to work independently.
• Ability to participate in a multi-disciplinary team.
• 2 years' experience in behavioral health or related field.

Responsibilities

• Evaluate members' needs and eligibility and plan care.
• Identify high-risk factors and refer to clinical management.
• Coordinate and monitor care plan activities.
• Collaborate with Medical Directors to improve care quality.
• Escalate quality of care issues per policy.
• Coach members to engage in healthy lifestyle choices.
• Assist with appointment scheduling and benefit access.
• Document care activities per regulatory guidelines.
