Job summary

CVS Health is seeking a Case Management Coordinator in Illinois to collaborate with the Aetna team. You will evaluate members’ needs, coordinate care plans, and navigate benefits and education resources to support complex health scenarios.

The role involves 40 hours weekly, travel of 50–75% within Winnebago, Boone, and McHenry counties, and requires a bachelor’s in behavioral health or related field. Excel and Word literacy are essential for success.

Qualifications

• Must reside in Illinois and travel 50–75% within Winnebago, Boone and McHenry counties.
• Strong computer literacy in internal/external systems including Excel and Word.
• Excellent communication, telephonic and organizational skills.
• Analytical and problem-solving capabilities; able to work independently.
• Ability to collaborate in a multi-disciplinary team with internal and external participants.

Responsibilities

• Evaluate referred members’ needs/eligibility and recommend case resolution approaches.
• Identify high risk factors and appropriate referrals to clinical case management.
• Coordinate and monitor assigned care plan activities.
