Responsibilities

• Evaluate member needs, eligibility and plan care using internal tools.
• Identify high-risk factors and refer to other care teams as needed.
• Coordinate and monitor care plan activities and progress.
• Consult with Medical Directors to improve care quality.
• Escalate quality of care issues through established channels.
• Assist members with accessing benefits and scheduling services.
• Coach members to engage in healthy lifestyle decisions.
• Document care activities and outcomes in required systems.
