• Excellent analytical and problem-solving skills.
• Strong communication, organizational, and interpersonal skills.

Responsibilities

• Coordinate appointment scheduling and assist members with accessing benefits and education through care management tools.
• Collaborate with members, providers and community organizations to address health care and social determinants of health needs.
• Support case management processes to inform healthcare outcomes and appropriate referrals.
