Job summary

CVS Health is seeking a Case Management Coordinator for the Aetna team in Illinois. The role supports members enrolled in Medicare and Medicaid, coordinating scheduling, benefits access and education through care management tools.

Responsibilities include evaluating needs, identifying risks, coordinating care plans, and promoting engagement with providers. This is a full-time position with comprehensive benefits and remote-friendly options.

Qualifications

• Experience in case management and discharge planning is preferred.
• 2 years in behavioral health, social services, or related field.
• Managed Care experience is preferred.
• Ability to travel within a geographic area for in-person activities.
• Strong analytical and problem‑solving skills.
• Excellent written and verbal communication; able to work independently.

Responsibilities

• Evaluate members' needs and review benefits to plan appropriate care.
• Identify high-risk factors and coordinate referrals to appropriate services.
• Coordinate and monitor assigned care plan activities.
• Collaborate with care teams to improve quality and outcomes.
• Escalate quality-of-care issues through proper channels.
• Use negotiation and motivational interviewing to secure services.
• Empower members to participate in healthcare decisions and healthy living.
• Participate in team meetings and provide peer mentoring.
