Job summary

Molina Healthcare in Illinois is seeking a Care Manager RN to support care management activities and coordinate member care across the continuum. This role focuses on ensuring member progress toward outcomes and aligning with strategic care delivery.

You will assess, plan, and implement care coordination with members, caregivers, physicians, and the care team. Travel to member sites is 25–40% as required by state contracts.

Qualifications

• Active RN license in state of practice.
• Minimum 2 years health care or care management experience.
• Valid driver’s license and reliable transportation.
• Knowledge of EMR and HIPAA.
• Strong communication and time management skills.
• Ability to work independently across settings.
• Proficiency with Microsoft Office and online portals.

Responsibilities

• Complete comprehensive member assessments per regulated timelines.
• Develop and implement care coordination plans with member, family, and care team.
• Conduct telephonic, in-person, or home visits as needed.
• Monitor care plans, document interventions and outcomes.
• Maintain ongoing member caseload with regular outreach.
• Coordinate services across behavioral health, LTSS, and home/community resources.
• Facilitate ICT meetings and interprofessional collaboration.
• Educate and motivate members using motivational interviewing.
• Identify barriers to care and assist members accordingly.
• May provide consultation to peers.
• Care manager RNs may handle complex cases and medication reconciliation.
• Travel 25-40% may be required.
