Job summary

Molina Healthcare seeks an experienced RN Care Manager to support care coordination for members across the continuum. You will collaborate with a multidisciplinary team to ensure progress toward outcomes and align with strategic goals for quality and cost-effective care.

The role involves assessments, care planning, travel up to 40% as required, and ongoing monitoring with strong emphasis on communication, documentation, and patient-centered interventions.

Qualifications

• At least 2 years experience in health care, preferably in care management, or equivalent combination of relevant education and experience.
• Active RN license in state of practice.
• Valid driver's license with reliable transportation for travel requirements.
• Understanding of EMR/EHR and HIPAA.
• Ability to work independently with attention to detail and adapt to diverse settings.

Responsibilities

• Completes comprehensive member assessments per regulated timelines and identifies who may qualify for care management.
• Develops and implements care coordination plan with member, family, physician and team.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness, documents interventions, and suggests changes.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of services across behavioral health, LTSS, and home/community resources.
• Facilitates interdisciplinary care team (ICT) meetings and collaboration.
• Uses motivational interviewing and Molina guidelines to support change.
• Assesses barriers to care and provides assistance to address concerns.
• May provide consultation and recommendations to peers as needed.
• Care manager RNs may handle complex cases and medication reconciliation.
