Qualifications

Responsibilities

• Completes comprehensive member assessments per timelines and determines care management eligibility.
• Develops and implements coordinated care plans with member, caregivers and providers.
• Performs telephonic, in-person or home visits as required.
• Monitors care plan effectiveness, documents interventions and adjusts as needed.
• Maintains ongoing member caseload and outreach.
• Facilitates ICT meetings and collaborates with care teams.
• Educates and motivates members using motivational interviewing techniques.
• Assesses barriers to care and provides resources to address concerns.
• May provide consultations to peers and conduct medication reconciliation as needed.
• Travel 25-40% may be required based on state/contractual requirements.
