Qualifications

Responsibilities

• Perform comprehensive member assessments within regulatory timelines.
• Develop and implement care coordination plans with the care team and family.
• Conduct telephonic, in-person, or home visits as needed.
• Monitor care plan effectiveness and adjust interventions.
• Maintain a caseload with proactive member outreach.
• Promote integration of behavioral health, LTSS, and community resources.
• Lead or participate in ICT meetings and care planning sessions.
• Utilize motivational interviewing to support member changes.
• Identify barriers to care and coordinate solutions.
• May provide peer consultation and resources as needed.
• Travel up to 25–40% based on state requirements.
