Job summary

Molina Healthcare is seeking an experienced RN care manager to support care coordination and multidisciplinary collaboration across the member care continuum. You will assess, plan, and monitor care plans, conduct visits, and evolve strategies to achieve outcomes while navigating HIPAA and EMR systems.

Qualifications

• 2+ years experience in health care, preferably care management
• RN license active and unrestricted
• Valid driver’s license and reliable transport for travel
• Understanding of EMR and HIPAA
• Knowledge of community resources
• Ability to work independently and manage multiple tasks
• Strong communication skills, both written and verbal

Responsibilities

• Supports care management/care coordination across multidisciplinary team
• Develops and implements care coordination plans with member and care network
• Performs telephonic, in-person, or home visits as required
• Monitors care plans, documents interventions and goals
• Maintains ongoing member caseload and outreach
• Promotes integration of behavioral health, LTSS, and community resources
• Facilitates ICT meetings and collaboration
• Assesses for barriers to care and provides assistance
• May provide consultation to peers
• Care manager RNs may handle complex cases and medication reconciliation
• Travel 25-40% as required
