Job summary

Qualifications

• Active RN license in state of practice.
• At least 2 years in health care or care management.
• Valid driver's license and reliable transportation.
• Knowledge of EMR and HIPAA.
• Strong communication and time management.
• Ability to work independently in diverse settings.
• Strong problem-solving and critical-thinking skills.
• Proficiency with Microsoft Office.

Responsibilities

• Complete comprehensive member assessments per timelines and identify candidates for care management.
• Develop and implement care coordination plans with the member and health professionals.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plan effectiveness and document outcomes.
• Maintain ongoing member caseloads and outreach.
• Promote integration of services across care continuum.
• Facilitate interdisciplinary care team meetings and collaboration.
• Educate and motivate members using motivational interviewing.
• Address barriers to care and provide coordination assistance.
• May provide consultation to peers.
• Handle complex cases and medication reconciliation as needed.
• Travel 25-40% may be required.
