Expect to perform assessments, develop care plans, and conduct home visits as required. The position requires active RN licensure, strong communication skills, and the ability to navigate EMR systems while ensuring HIPAA compliance.

Qualifications

• Must be a licensed RN with active, unrestricted licensure in the state of practice.
• Experience in care management or medical/behavioral health setting preferred.
• Knowledge of HIPAA and EMR systems.
• Strong verbal and written communication skills; ability to collaborate with multidisciplinary teams.
• Willingness to travel locally (approx. 25-40%).

Responsibilities

• Assess members per timelines and determine eligibility for care management.
• Develop and implement care coordination plans with care team and family.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and adjust care plans based on progress and barriers.
• Maintain ongoing member caseload and coordinate services across settings.
• Facilitate ICT meetings and collaborate with care teams.
• Provide education and motivational interviewing to support behavior change.
• Ensure documentation of interventions and outcomes in EMR.
• Support peers with resources and coordination as needed.
• May handle complex cases and medication reconciliation.
