Job summary

UnitedHealthcare is seeking a Care Coordinator to coordinate Medicare/Medicaid member care needs with a focus on complex cases. The role supports access to services, education, and proactive care planning across acute, home, and community settings.

The position requires a Bachelor’s in a health care field, 2+ years of case management, and willingness to travel up to 75% within the territory. Remote work options available within Haywood County, TN.

Qualifications

• Bachelor's degree in social work, nursing or health care profession including LPN.
• 2+ years of clinical case management experience.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Strong organizational, communication and time management skills.
• Reside in or near Haywood County, TN.
• Access to reliable transportation and valid US driver's license with travel up to 75% within an assigned territory.

Responsibilities

• Apply clinical knowledge to manage cases for Medicare/Medicaid members.
• Deliver self-management education and patient education.
• Coordinate services across care settings to meet member needs cost-effectively.
• Use Health Plan policy for assessments and document accordingly.
• Collaborate with Care Coordination team to improve care quality.
• Enter service requests and monitor all aspects of member care coordination in Health Plan software.
