Job summary

Humana Inc. in Illinois is seeking a Care Coordinator to support Medicaid Members in the MLTSS/LTSS tracks. You will assess needs, develop personalized care plans, and connect members and families with community resources to improve health and well-being.

The role requires residence in Illinois, 1+ year experience in health care or case management, and familiarity with Medicare/Medicaid services and electronic medical records. Schedule is Monday–Friday 8am–5pm CST with travel 75–90%.

Qualifications

• Must reside in Illinois.
• 1+ years of experience in health care and/or case management.
• 1+ years of experience with Medicare & Medicaid recipients and/or long-term care and/or Home and Community based services and/or working with managed care organizations.
• Intermediate to advanced computer skills and experience with Microsoft Word, Excel, and Outlook.
• Experience using a variety of electronic information applications/software programs including electronic medical records.

Responsibilities

• Visit Medicaid members in homes, facilities or care settings with 75%-90% local travel.
• Assess care needs and ensure services are in the least restrictive setting.
• Identify and resolve barriers to effective care.
• Plan and implement interventions to meet care needs.
• Coordinate services, monitor and evaluate the care plan against member goals.
• Monitor patient care to ensure progress toward outcomes.
• Assess enrollee needs via telephonic and face-to-face assessments and evaluations.
• Assist enrollee to remediate immediate gaps in care and access.
• Assist enrollee with filing grievances and appeals.
• Provide information on MCO requirements, services and benefits.
• Provide referrals to community resources.

Education

Bachelor's degree in social sciences, social work, human services, or related field

Unrestricted Licensed Practical Nurse (LPN) in Illinois

Licensed Professional Counselor (LPC)

Licensed Social Worker (LSW)
