Humana is seeking a Care Coordinator to support Illinois HealthChoice Medicaid Managed Care. You will assess member needs, create care plans, and connect families with resources. This field role requires 75–90% travel and residence in Illinois, with standard 40-hour workweeks.

Responsibilities include home visits, coordination across providers, and assisting with grievances and access to benefits. A relevant license or degree is required.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or related field.
• LPN in Illinois with 1 year assessing elderly clients.
• Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW).

Responsibilities

• Visit Medicaid members in homes, facilities, or other settings; 75-90% local travel.
• Assess needs and develop personalized care plans.
• Coordinate services and monitor care plans against goals.
• Assist with grievances, appeals, and accessing community resources.

Education

Bachelor's degree in social sciences, social work, human services, or related field

LPN in Illinois with 1 year experience in assessments

Licensed Professional Counselor (LPC)
