Job summary

Humana Inc in Illinois is seeking a Care Coordinator to support Medicaid members in the MLTSS/LTSS programs. You will visit members in homes and care facilities, assess needs, develop care plans, and connect families with resources.

This field role requires 75–90% local travel, IL residency, and 1+ year experience in health care or case management. You will collaborate with interdisciplinary teams and navigate Medicare/Medicaid services.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or related field.
• Unrestricted LPN in Illinois with 1 year of assessment experience.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).
• Must reside in Illinois.
• 1+ year experience in health care and/or case management.
• 1+ year experience with Medicare & Medicaid or long-term care or managed care.
• Intermediate to advanced computer skills (Word, Excel, Outlook).
• Experience with electronic medical records.

Responsibilities

• Visit Medicaid members in homes, Assisted Living Facilities, and/or Long-Term Care Facilities; 75-90% travel.
• Assess care needs and plan interventions to meet them.
• Coordinate services, monitor, and evaluate the care plan against goals.
• Assist enrollee with filing grievances and appeals.
• Provide information on MCO requirements, services, and benefits.
• Refer enrollees to community resources.
• Guide families toward resources and coordinate with payers, providers, and interdisciplinary teams.

Education

Bachelor's degree in social sciences, social work, human services, or related field

Unrestricted Licensed Practical Nurse (LPN) in the state of Illinois with one year experience conducting comprehensive assessments and provision of formal service for the elderly

Licensed Professional Counselor (LPC)

Licensed Social Worker (LSW)
