Job summary

Humana is seeking a Care Coordinator for Illinois Medicaid members. You will assess needs, develop personalized care plans, and connect members with resources to thrive, reporting to the Care Management Manager.

The role requires extensive field travel (75-90%), and involves navigating services, coordinating with care teams, and addressing enrollee needs. The position is full-time and located in Illinois, with travel and occasional remote work supported by Humana’s field operations.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or a related field.
• LPN license in Illinois with 1 year experience in assessments for the elderly.
• Licenses as LPC or LSW.

Responsibilities

• Visit Medicaid members in homes, assisted living, and long-term care facilities, with 75-90% travel.
• Assess care needs and ensure services in least restrictive settings.
• Identify and resolve barriers hindering effective care.
• Plan and implement interventions to meet care needs.
• Coordinate, monitor, and evaluate the care plan against personal goals.
• Monitor progress toward outcomes and adapt care plans accordingly.
• Assess enrollees via telephonic and in-person assessments to coordinate services.
• Assist enrollees to remediate care gaps and access issues.
• Assist enrollees with grievances and appeals.
• Provide information about MCO requirements, services, and benefits.
• Refer enrollees to community resources and guide families to appropriate services.

Education

Bachelor's degree in social sciences, social work, human services, or a related field

Unrestricted Licensed Practical Nurse (LPN) in Illinois

Licensed Professional Counselor (LPC)
