Job summary

Humana is seeking a Care Coordinator for Illinois Medicaid members within the HealthChoice Medicaid Managed Care program. The role involves assessing needs, developing care plans, and connecting families with resources to improve health outcomes.

The position requires Illinois-resident, 1+ year healthcare/case management experience, and ability to travel 75–90% locally. A strong background in case management and EHR systems is preferred.

Qualifications

• The Care Coordinator must meet one (1) of the following requirements: Bachelor's degree in social sciences, social work, human services, or a related field.
• Unrestricted LPN in Illinois with one (1) year experience conducting comprehensive assessments and provision of formal service for the elderly.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).

Responsibilities

• Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long-Term Care Facilities and other care settings – 75%-90% local travel.
• Assess care needs to ensure services are provided in the least restrictive setting.
• Identify and resolve barriers hindering effective care.
• Plan and implement interventions to meet care needs.
• Coordinate services, monitor, and evaluate the care plan against member goals.
• Monitor patient progress toward outcomes via assessments and care planning.
• Assess enrollees' needs via telephonic and face-to-face assessments to coordinate services.
• Assist enrollees to remediate gaps in care and access.
• Assist enrollees with grievances and appeals.
• Provide information on MCO requirements, services, and benefits.
• Refer enrollees to community resources.
• Guide families toward resources and coordinate with other providers and payer sources.

Education

Bachelor's degree in social sciences, social work, human services, or a related field

Unrestricted Licensed Practical Nurse (LPN) in the state of Illinois with one (1) year experience

Licensed Professional Counselor (LPC)

Licensed Social Worker (LSW)
