Job summary

Humana is seeking a Care Coordinator for Illinois HealthChoice Medicaid Managed Care. You will assess member needs, develop care plans, and connect families with needed resources. This field role requires 75-90% travel to members’ homes or care facilities and collaboration with interdisciplinary teams.

The role supports MLTSS or LTSS, reports to Care Management, and emphasizes timely, person-centered care coordination with a strong understanding of Medicaid requirements.

Qualifications

• 1+ years of experience in health care and/or case management.
• Experience with Medicare & Medicaid recipients and/or long-term care and/or Home and Community based services and/or managed care organizations.
• Intermediate to advanced computer skills and experience with Microsoft Word, Excel, and Outlook.
• Experience using electronic medical records.

Responsibilities

• Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long-Term Care Facilities and other care settings – 75%-90% local travel.
• Assess member needs and develop personalized care plans.
• Coordinate services and monitor the care plan against member goals.
• Identify and resolve barriers to effective care and access.
• Assist enrollment with grievances and appeals.
• Provide information on services, benefits, and community resources.
• Guide families to resources appropriate for care.
