Job summary

Humana is seeking a Care Coordinator in Illinois to support the Medicaid Managed Care program. You will assess member needs, develop care plans, and connect families with resources to improve health outcomes.

This field role requires substantial travel and collaboration with multidisciplinary teams to ensure timely services and adherence to program requirements. Reporting to the Manager of Care Management, you will work to enable members to achieve their best health while navigating benefits,

Qualifications

• Bachelor’s degree in social sciences, social work, human services or related field.
• Unrestricted LPN in Illinois with 1 year of elderly services experience.
• Licensed Professional Counselor (LPC) certification.
• Licensed Social Worker (LSW) certification.

Responsibilities

• Visit Medicaid members in homes, assisted living facilities or other care settings; 75–90% travel.
• Assess needs and ensure services in least restrictive settings to optimize wellbeing.
• Identify and resolve barriers hindering effective care.
• Plan and implement interventions to meet care needs.
• Coordinate services and monitor the care plan against personal goals.
• Ensure progress toward outcomes via assessment, data review, and conversations.
• Assess enrollee needs via telephonic and face-to-face evaluations to coordinate services.
• Assist with removing gaps in care and access, including grievances and appeals.
• Provide information on MCO requirements, services and benefits.
• Refer enrollees to community resources and guide families to appropriate supports.
