Job summary

Humana is seeking a Care Coordinator in Illinois to support the Medicaid HealthChoice program. You will assess members’ needs, build personalized care plans, and connect them with resources to improve health outcomes.

The role reports to the Manager, Care Management and requires substantial field travel to visit members in various settings. The position requires a valid Illinois license (LPN/LCSW/LPC or similar) and 1+ year of health care/case management experience, with strong computer skills

Qualifications

Bachelor's degree in social sciences, social work, human services, or a related field.
Unrestricted LPN license in Illinois with one year experience.
Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW).
Residence in Illinois and 1+ years experience in health care/case management.

Responsibilities

Visit Medicaid members in homes, Assisted Living Facilities, and Long-Term Care Facilities with 75%-90% travel.
Assess care needs to ensure services in the least restrictive setting.
Identify and resolve barriers hindering care.
Plan and implement interventions to meet care needs.
Coordinate services and monitor the care plan against member goals.
Monitor patient progress and adjust care plans as needed.
Assess enrollee needs via phone and in-person assessments to coordinate services.
Assist enrollee with addressing gaps in care, grievances, and appeals.
Provide information on MCO requirements, services, benefits, and community resources.
