Job summary

Humana seeks a Care Coordinator to support Illinois HealthChoice Medicaid Managed Care. You will assess members, create personalized care plans, and connect families to necessary resources to improve health outcomes.

The role involves 75%-90% local travel and a field-oriented schedule, with 40 hours weekly. Reporting to the Manager, Care Management, you will work in a remote-capable environment while traveling to member sites as needed and aligning with Humana’s TB and driver-safety programs.

Qualifications

• The Care Coordinator must meet one (1) of the following requirements.

Responsibilities

• Visit Medicaid members in their homes, Assisted Living Facilities, and Long-Term Care Facilities with 75%-90% local travel.
• Assess member needs and develop personalized care plans.
• Identify and resolve barriers hindering care.
• Plan and implement interventions to meet care needs.
• Coordinate services and monitor the care plan against member goals.
• Ensure progress toward outcomes through ongoing assessment and communication.
• Assess enrollee needs via telephonic and in-person evaluations to coordinate services.
• Assist enrollees with immediate gaps in care and access.
• Assist enrollees with filing grievances and appeals.
• Provide information about MCO requirements, services, and benefits.
• Refer enrollees to community resources.
• Guide families to resources appropriate for care; coordinate with providers and interdisciplinary teams.

Education

Bachelor's degree in social sciences, social work, human services, or a related field

Unrestricted Licensed Practical Nurse (LPN) in Illinois

Licensed Professional Counselor (LPC)

Licensed Social Worker (LSW)
