Job summary

Humana is seeking a Transition/Custodial Prevention Specialist in Illinois to work one-on-one with members, creating personalized care plans, coordinating services, and empowering better health outcomes and independence.

In this remote field role, you will meet members at home, nursing facilities, or locations of their choice, focusing on LTSS, HCBS, and connecting them with community resources. Travel 75-90% and occasional meetings in Schaumburg may be required.

Qualifications

• Must reside in Illinois.
• Two years of experience with Medicare/Medicaid, HCBS, or managed care.
• Experience with custodial and transitional care.
• Ability to travel regionally to meet members and families.
• Experience with electronic medical records or electronic information systems.

Responsibilities

• Develop and modify Individual Service Plans with care team involvement.
• Coordinate LTSS and connect members with HCBS and community services.
• Provide psychosocial support and advocate for member care and access to services.
• Travel to meet members in their homes or facilities and assess needs.
• Educate members on Medicaid eligibility and available benefits.
• Collaborate with providers, payers, and interdisciplinary care teams.
