Job summary

Qualifications

• Reside in Illinois.
• 2 years of experience with Medicare/Medicaid recipients, long-term care, HCBS, or managed care.
• 1+ year of experience with custodial and transitional care.
• Experience using electronic information applications/software including EMR.
• Able to travel regionally to meet members and care teams.

Responsibilities

• Meet members in the field (home, NF, or chosen location) to assess goals and plan LTSS.
• Develop plans of care and identify LTSS providers and community partnerships.
• Coordinate services across disciplines to address health, social, housing, and educational needs.
• Keep members informed and navigate Medicaid eligibility and service options.
